Navigating Dosage and Administration

  • Enhancing Absorption:
    • For optimal effectiveness, the medication should be held under the tongue for 1-2 minutes before swallowing.
  • Ideal Timing
    • Administer first thing in the morning, 30 minutes before any other medications or food/beverages, to maximize absorption. Remember to shake well before use.

  • It's crucial to remember that the journey with oral semaglutide is highly personalized. Dosing schedules are adaptable to each individual’s response, with flexibility in titration speed based on side effects and tolerability. For some, this might be the full 1mg (1ml) daily dose, but your journey might look different—and that's perfectly okay.

Initial Phase (Month 1): You will receive a 15ml vial (31 day supply)

  • Start your journey with an introductory dose to gently acclimate your body to semaglutide. By incrementally increasing your dose it will help to decrease the potential for side effects and allow your body to adjust to the medication.
    • Week 1:   Place 0.3ml under the tongue every morning.
    • Week 2 & 3: Increase to 0.5ml every morning.

  1. Self-Administration: A Simplified Process
    • Shake the Vial
      • Before each use, ensure you shake the vial vigorously. This ensures the medication is well-mixed and consistent in concentration.
    • Measuring Your Dose
      • Use the Provided Syringe: Draw the prescribed amount of oral semaglutide into the oral syringe provided. If unsure about the exact measurement, refer back to your prescription details or consult your healthcare provider.
    • Administration
      • Lift Your Tongue: Gently lift your tongue and place the tip of the oral syringe under it. Slowly press the plunger of the syringe to dispense the medication under your tongue. Try to distribute it evenly.
      • Keep the medication under your tongue for 1-2 minutes. The longer it stays, the better the absorption through the sublingual mucosa.
      • After waiting, you can swallow any remaining medication.
    • Post Administration
      • Avoid Food and Drink: Wait at least 30 minutes before consuming any food, beverages (other than water), or other oral medications. This waiting period ensures the medication is absorbed effectively without interference.
    • Storage:
      • Store the vial at room temperature, away from direct sunlight and moisture, ensuring it's ready for your next dose.
  2. What happens if I miss a dose?
    • If you miss a dose of this medicine, skip the missed dose and go back to your regular dosing schedule. Do not double dose.
  3. Can I take semaglutide while pregnant or breastfeeding?
    • No studies have been performed to determine safety of semaglutide in patients who are pregnant or breastfeeding.
    • Semaglutide should be discontinued for ≥2 months prior to becoming pregnant.
  4. What if I need a higher dose?
    • If you find that you might need a higher dose of oral semaglutide, it's essential to consult with your healthcare provider. They can assess your progress, side effects, and overall health to determine if adjusting your dose is appropriate. Remember, any changes to your medication should only be made under the guidance of a healthcare professional to ensure your safety and the effectiveness of your treatment.

  13. What type of quality assurance measures do you use?

Special considerations:

  • Must take upon waking in the mornings.
  • Taking with any food, beverages, or other medications can reduce the effectiveness of this drug and alter the absorption of any other drugs.
  • Do not take with Levothyroxine or any other medications with a narrow therapeutic index. Separate at least 30 minutes from this or any other medications.
  • If you are on Levothyroxine, or any other medications with a narrow therapeutic index, such as aminoglycosides, ciclosporin, carbamazepine, digoxin, digitoxin, flecainide, lithium, phenytoin, phenobarbital, rifampicin, theophylline or warfarin—speak with your doctor to see if this medication may be right for you.

Most common side effects include:

  • Nausea

  • Decreased appetite

Do not use if you have been diagnosed with the following:

  • Diabetic ketoacidosis
  • Type 1 diabetes
  • Diabetic retinopathy
  • Kidney disease
  • Multiple endocrine neoplasia syndrome type 2 (MEN 2)
  • Thyroid cancer
  • Pancreatitis

SEMAGLUTIDE WARNING

  • Please contact your doctor ASAP and seek in-person care immediately if a side effect becomes problematic or you experience:
    Severe nausea and/or vomiting resulting in dehydration—it is important to stay well hydrated and drink plenty of fluids while on this medication.
  • Kidney problems/kidney failure:
    • vomiting, nausea, diarrhea can cause dehydration and kidney issues and make kidney problems worse in individuals who have pre-existing kidney conditions; may sometimes lead to the need for hemodialysis.
  • Thyroid C-Cell Tumor:
    • Trouble swallowing, hoarseness, a lump or swelling in your neck, or shortness of breath (see black box warning below).
  • Pancreatitis:
    • Severe pain in your abdomen or back that will not go away.
  • Acute gallbladder disease:
    • Pain in the middle or right upper stomach, fever, white parts of your eyes turn yellow or skin turns yellow, nausea/vomiting. If gallstones or inflammation of the gallbladder is suspected further studies are needed.
  • Hepatitis/Elevated liver enzymes/Jaundice.
  • Diabetic retinopathy problems: Changes in vision in those with type II diabetes, particularly vision problems may worsen in those with a history of diabetic retinopathy.
  • Severe gastrointestinal disease—may cause gastrointestinal disease; do not use if you have a history of severe gastrointestinal disease.
  • Signs of low blood sugar (for example, dizziness, weakness, shakiness, jitteriness, headache, anxiety/mood changes, rapid heartbeat, confusion, slurred speech, sweating, or lightheadedness). While this is rare with GLP-1s, if it occurs, please seek in-person care immediately. If you have these symptoms, try having liquid or food containing sugar (around 15-20 grams)—drinking juice, having regular soda, or eating some hard candies right away, as this will help raise your blood sugar quickly. If taken with insulin or an insulin secretagogue (ie sulfonylurea) it is best to talk to your healthcare provider about possibly lowering the dose of these medications to lower the risk of hypoglycemia.
  • Increasing heart rate.
  • Worsening depression, behavior changes, or suicidality.
  • Never share the semaglutide with other people.
